Unless clearly specified otherwise, each applicant will be deemed to request one tree. If more than 3 trees are requested, please provide details regarding the landscape plan in an addendum to this application. Requests for more than 3 trees per applicant are subject to review and approval by the City's steering committee for its Beat the Heat----Plant a Tree, Columbia Tree Giveaway Program ("Program").

I agree that neither the City of Columbia nor any participant in the Beat the Heat Program, nor any volunteer person or organization assisting with education, planting or maintaining the trees provided under the Program ("Program Trees), shall be liable for any injury or damage to persons or property resulting from the delivery, planting, maintenance or growth of Program Trees. I further agree that, with respect to any and all Program Trees which I accept, it is my sole responsibility to evaluate the proper location for planting and to provide water, pruning and other care necessary for proper maintenance.

I have reviewed the educational materials (Tree Planting Instructions, Tree Planting Guide & Choosing the Right Tree), have been provided an opportunity to ask questions of Program Participants and have had the opportunity to conduct my own research in connection with the planting and maintenance of the Program Trees which I accept. Accordingly, I hereby agree to accept the Program Tree(s) specified by me as preferred in Item 1 above and to plant such Program Tree(s) on the property specified in this Application, which I hereby certify that I own or with respect to which I have the legal right to plant trees. I further agree that I shall not offer any Program Tree for resale.
