  • Volunteer/Foster Waiver,
    Acknowledgement of Risk, and
    Release of Liability

  • I, the above-named volunteer, enter into this Volunteer Waiver, Acknowledgement of Risk, and Release of Liability (hereafter, “Waiver”) with the City of Columbia Animal Services (hereinafter, “CAS”) to permit me to provide volunteer services. This Waiver is for the benefit of all parties, particularly the City of Columbia, CAS and its staff members, affiliates, employees, officers, directors, and representatives (known individually as an “indemnitee” and collectively as “indemnitees”).

    By entering into this Waiver, I acknowledge that working with shelter animals is hazardous and involves contact with animals that are unpredictable. I further acknowledge that volunteering can involve contact with other volunteers, the general public, and employees of and animals at other entities (e.g., veterinarian
    offices, animal transport organizations). I acknowledge that there are risks of physical or psychological injury, damage to personal property, and economic loss, which can include, but are not limited to, the following:

    Physical injury from animals, property damage from animals, physical/property damage from hazards in the shelter environment, zoonotic disease exposure, offsite disease transfer, general injury/damage, exposure to other humans.

    I am willfully participating in volunteer activities with CAS and am participating entirely at my own risk. I am aware of the risks associated with these activities and have made the conscious decision to assume all related risks, both known and unknown to me, of my participation in these activities. I agree to indemnify and hold harmless each indemnitee against any and all claims, suits, or actions of any kind whatsoever for liability, damages, compensation, or otherwise brought by me or anyone on my behalf, including attorney’s fees and any related costs of litigation if the same arises pursuant to any claims made by me or anyone else acting on my behalf.

    I acknowledge that I have carefully read this document prior to signing and that I had an opportunity to have it reviewed by an attorney of my choice. I further agree that jurisdiction and venue shall be proper only in Richland County, South Carolina for any dispute arising from the activities described herein or from this agreement, and that South Carolina law shall apply. If any provision of this Waiver is found to be unenforceable in any way, it shall be enforced to the maximum extent possible and all other provisions of this Waiver shall remain in full force and effect.

    I represent and warrant that I am physically and mentally fit to safely work with animals and public at the shelter. I represent and warrant that I have the authority to enter into this agreement, whether on my own behalf or for my child(ren) or ward. I agree to follow all rules, signage, and directions given by CAS.

  • Volunteer/Foster Code of Conduct
    Agreement

  • I, the above-named volunteer, enter into this Code of Conduct Agreement
    (hereafter, “Agreement”) with the City of Columbia Animal Services (hereafter,
    “CAS”) in order to participate in the volunteer program at CAS. I recognize the
    below requirements and agree to be bound by them during the performance of my
    duties as a volunteer.

    I will act reasonably and make informed decisions that further the overall best
    interest of CAS. This includes being conscious of potential conflicts of interest with
    other organizations. I agree to treat all matters I observe or in which I engage at
    CAS as confidential unless and until public disclosure has been made.

    I recognize that, as a volunteer, I am acting as a representative of CAS to external
    organizations and the public at large. As such, I will act responsibly, respectfully,
    and professionally. I will use good judgment while volunteering. I realize that I do
    not have the authority to bind CAS to any agreement or obligation, whether legal or
    otherwise.

    I recognize that I am not an employee and provide my time and effort without any
    expectation of recompense, remuneration, or reimbursement.

    I will inform CAS of any felony charges, restrictions, suspensions, or revocations of
    any licenses that may negatively affect CAS or impair my continued ability to
    volunteer. I understand that, after review with appropriate staff, such disclosures
    may lead to termination or limitation of my volunteer activities with CAS.

    Regardless, CAS volunteers serve on an “at will” basis and both CAS and I may
    terminate the volunteer relationship at any time, with or without notice or reason.
    Because I am not an employee, I am not covered by or entitled to any workers’
    compensation protections.

    I agree that I will not take or use any unauthorized photographs or videos from the
    shelter or any volunteer activity in which I participate. I further agree that I will not
    disseminate or post any unauthorized information on social media or release such
    information about CAS to the public.

    I agree not to remove, duplicate, or share any intellectual property or work product
    that I may create or use on behalf of CAS.

    I agree to follow all rules, signage, and directions given by CAS.
